	At the end of the rotation, submit your portfolio.   Your performance on this rotation will be evaluated through your written and verbal answers to the questions and on your reflection of the experiences.  You will be assessed on 6 ACGME competencies, specific to this rotation. Faculty will evaluate your competencies through the scheduled face-to-face meetings , e-mail submissions, and your portfolio entries. You will also be asked to submit evaluation of faculty through MyUTMB.   You are responsible for completing the following portfolio entries: □ e-mail 14 modules (sent to Niebuhr and other faculty):              no need to include hard copy in the portfolio □ Rotation Summary Evaluation  include hard copy in portfolio and also e-mail as attachment □ Developmental screenings Portfolio Entry: Developmental Screening              include hard copy in portfolio and also e-mail attachment Developmental screening log Denver II scoresheet copies (originals should have gone to medical records) collection of Draw-a-Person's for patients over 3 □ Procedure Log. Documentation of developmental screenings is required in the procedure log for your residency program. Suggestion: take the time during this rotation to enter these developmental screenings into your procedure log.                                         Procedure Log (New Innovations)  □ Pairings with a Pediatrician. if complete,  include hard copy in portfolio, and also e-mail as attachment.
