If you do not bring these items with you to
Benefits Services, your paperwork will NOT be processed:
ALL names on ALL documents must match exactly,
including the name on your professional license RN, MD, RPH ….

New Employees:
The hiring process cannot be completed until you
visit Benefits Services and the Employee Health Clinic.
Upon completion of the above mentioned processes, you will
receive final clearance to go to work, including:
- Issuance of ID Badge - for any degree(s) listed after your name, you must provide the police
department with your credentials at the time your badge is made.
