All new employees must successfully complete their visit to Benefits Services BEFORE they begin working.

Parking is available in the garage located at the corner of Market and University Blvd. You are responsible for parking fees.

WHAT TO BRING WITH YOU…

If you do not bring these items with you to Benefits Services, your paperwork will NOT be processed:

ALL names on ALL documents must match exactly, including the name on your professional license RN, MD, RPH ….

  • Completed New Employee Packet (Adobe PDF)
  • Photo ID (for ID Badge Issuance)
  • Application/Resume/or CV (Curriculum Vitae)
  • Social Security Card (for payroll verification) US Social Security Office is located at:

New Employees:

The hiring process cannot be completed until you visit Benefits Services and the Employee Health Clinic.

Upon completion of the above mentioned processes, you will receive final clearance to go to work, including:

  • Issuance of ID Badge - for any degree(s) listed after your name, you must provide the police department with your credentials at the time your badge is made. Approved degrees/credentials
  • New Employee Orientation Date
  • Phone number to call for Employee Health Assessment
    (Clinical employees must bring immunization records to Employee Health Visit)
