For use of high risk hazardous chemicals complete the Safety Plan for High Risk Hazardous Chemical Use. Instructions to complete the form are included after the form.

For use of the Environmental Toxicology Lab complete the Safety Plan for the Use of the Environmental Toxicology Laboratory.

The forms must be reviewed and approved by the Chemical Safety Committee. Submit the completed form to EHS, B&C, route 1111, by the second week of each month.

 

All High Risk Hazardous Chemical users must provide EHS B&C with an annual update on their approved Safety Plans.  Please complete THE UTMB STATEMENT BY THE PRINCIPAL INVESTIGATOR FOR THE ANNUAL STATUS OF SAFETY PLAN FOR THE USE AND STORAGE OF HIGH RISK HAZARDOUS CHEMICALS.  One form must be completed for each approved Safety Plan.
