ECI Launch Ages & Stages Questionnaire

ASQ Online is a 10 minute voluntary parent-completed developmental screening. ECI Launch is pleased to offer this service to parents in Galveston County, Webster, Seabrook and Friendswood.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. The Ages & Stages Questionnaire will help you keep track of your child's development. Feel free to complete a new questionnaire every 6 months. You will be asked to answer questions regarding what your child can and cannot do. The ASQ includes questions about your child's communication, motor skills, problem solving, and personal social skills.

After completion of the ASQ, you will receive your child's screening results along with age appropriate activities to promote development, via email or USPS. If the ASQ indicates that your child is experiencing a developmental delay, a referral will be made to ECI Launch, the Early Childhood Intervention program in Galveston County for children birth to 36 months or to the local school district for children 3 to 5½ years of age. ECI and ISD developmental evaluations are offered to you at no cost for the benefit of your child and are also voluntary.

Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.
