In order to improve our customer service, we are implementing a new electronic roster system beginning September 15, 2008. It is our hope that this system will simplify roster submission and improve our turn around time for card processing. Please review the following changes and let us know if you have any questions.

1. New electronic roster forms for each of the AHA courses taught by the UTMB Training Center are available by email and at the UTMB Edlab Website. Please use these forms for each individual course and be sure to fill out all the fields.

2. After September 15, 2008, only rosters submitted electronically (via email) using the new electronic roster forms will be accepted. If you are not able to use the electronic roster form, please call me at 409-772-5513.

3. All rosters must be submitted to the following email address: ed.lab@utmb.edu this email address is for roster submission, requests for duplicate cards and roster corrections only.

4. As of September 15, 2008, cards must be either paid for in advance or paid for at the time of roster submission. All past due balances must be paid off before additional rosters will be processed. Training Sites may make deposits in advance to cover future roster submissions.

6. Card prices are as follows: $1.25 per Heartsaver/ Basic Life Support Card

$2.75 per Advanced Cardiac Life Support Card/Pediatric Cardiac Life Support Card
