UTMB Hot Work Basic Instructions


 

Before issuing any permits, make sure you have read the Hot Work Permitting Procedures , understand the requirements, and are listed as an Permit Authorizing Individual (PAI).

 

1. Visit the FOAM Intranet Maintenance Webpage
2. Click on Hot Work Permit:  this takes you to the main Hot Work Permit page
3. Click on Online Hot Work Permit:  a form should show up on your screen
4. Fill in all the information on the form and click submit
5. IMMEDIATELY print out the completed Permit;  you will NOT be able to re-print the permit without making a new permit
6. In the right hand column, under Required Precautions Checklist, select the appropriate precautions for the area the permit is issued for. Questions about what is appropriate should be directed to EHS at ext 70515
7.

Take the permit to the job site, inspect the site for confirmation of required precautions, and get the following signatures:

-     Yourself (as the PAI)

-     The job supervisor

-     The actual person performing the work;  get the signatures of any persons who may be conducting the work, expand onto the back of the permit if necessary

For the duration of the permit (up to 5 consecutive days), every day work continues update and initial the permit (middle of left column)
