UTMB Hot Work System Outage Procedures
| The following procedure shall apply when the system is experiencing outages and a Hot Work Permit can not be obtained through the usual online procedure | |
| 1. | Visit the FOAM Intranet Maintenance Webpage |
| 2. | Click on Hot Work Permit: this takes you to the main Hot Work Permit page |
| 3. | Click on Blank Permit for System Outages: a completely blank permit should appear |
| 4 | Print off the blank permit |
| 5. | Fill in all the fields on the permit |
| 6. | Immediately fax the permit to EHS at ext. 28501 |
| 7. | Complete the process as per the rest of the standard procedure |
