Harborside and 9th intersection Limited visibility for traffic and concerns of speeding vehicles 11/8/04 1/1/05 Harborside is City of Galveston and TxDOT property and UTMB does not have control over the road.
Harborside near Primary Care Pavilion Patients and employees walking to PCP must cross uneven ground 11/8/04 1/1/05 FOAM is forwarding this to TexDOT, as this area is under their jurisdiction.
Postoffice Street between 6th St and 8th St Sidewalk blocked by UTMB vehicles, blocking pedestrian access 11/8/04   In progress
12th and Strand Vehicles parked in crosswalks, blocking pedestrian access 11/8/04 11/12/04 The Safety Officer for the projects (UT System Office of Facilities Planning and Construction's project) has reminded contractors to not block any pedestrian crosswalks and will re-iterate the message in weekly construction meetings.
12th and Strand Traffic cones being moved, blocking traffic 11/8/04 11/30/04 EHS has not observed this happening since the report.
Parking Lot D Annex from Mechanic and 12th street Uneven walking surface 11/8/04 11/12/04 UTMB has a  project for updating ADA issues across campus.  This sidewalk area will probably be part of the project.

Around Plaza Project Bicyclists and people on foot are almost colliding in the narrow walkways used to get around campus construction 11/9/04 11/10/04 Signs are being posted:

Various locations Painted curbs are slippery when wet 11/11/04 1/1/05 EHS has requested that curbs be painted with non-slip paint.  We recommend walking to the corner of streets and using the ramps for getting to street level instead of stepping down curbs.
MRB Animals transported in elevators 11/8/04 11/15/04 UTMB has many research only buildings on campus, including MRB, and such buildings may have labs performing animal research. In such buildings, transportation of these animals is necessary for normal operations. While transportation in freight elevators, when available, is preferable, these animals may also be transported in the normal elevators. During transport proper animal husbandry measures must be followed to ensure the safety of all involved.
18th & Harborside Traffic at 5pm makes for difficulty in walking across Harborside 11/11/04   In progress

Jennie Sealy Hospital Steps near ramp are dark at night creating a trip hazard 11/11/04 12/2/04 Street lights in the area will be reactivated.  Additional measures will be taken if the streetlights do not provide enough additional lighting.
Harborside M-lot Driveway exit from the new parking lot and existing M-lot are very close with one way traffic.  Several near-miss collisions have been reported in this area 11/12/04   In progress
11th and Market After hours, employees are driving the wrong way down 11th to enter parking lots 11/16/04 11/16/04 New signage with No Left Turns should be posted until 11th is re-opened
New UHC covered walkway Pick up area for patients has a pebbled surface which is slippery during wet weather 11/16/04 12/22/04 A non-slip coating was applied when the area was first installed.  FOAM is looking at re-coating the area with a non-slip material.
9th and Market Construction barricades are not being maintained and drivers are driving wrong directions on closed sections of Market 11/16/04 11/17/04 While the area is closed to regular traffic, many construction workers are using that area to access construction sites and to move people from Gail Borden.
Hospital Parking Garage by Administration building Support posts blocks views of pedestrians for drivers and vice versa 11/16/04 11/16/04 Police officers have been posted in front of John Sealy Hospital and on the West side of the parking garage to help guide pedestrians and vehicle traffic in the area
Clinical Science 3rd floor Auditorium Steps at the back of the auditorium are hard to see when the lights are dim 11/16/04 12/3/04 FOAM is looking at various options and costs for adding some lighting along the steps to provide better visibility when the house lights are dimmed.
John Sealy Annex New tank bracket installed in drywall without necessary hardware or being  attached to a stud 11/29/04 12/1/04 E-mailed all Construction Managers and Maintenance Stewards to remind them of the need for secure mounting of the brackets to walls to ensure user safety.
Sidewalk between John Sealy Hospital and Shriners Burn Institute Sidewalk covered in bird feces being tracked into hospital 11/29/04 12/1/04 As per Glen Mayhall:

"To my knowledge, there have been no reports relating bird droppings outside the hospital to hospital-acquired infections. If a residue of bird droppings was deposited on the floors in hospitals from the shoes of pedestrians, it appears unlikely that such deposits would lead to infections in patients. There are no data in the literature to relate floor contamination to infections in patients. Such soil is removed by the routine cleaning of floors by Environmental Services. This situation should not be classified as a near miss."

West side of Jamail Student Center Trash dumpster left in walkway everyday 11/29/04 12/1/04 Environmental services has been notified and has notified employees in that area to make sure to leave the sidewalk clear.
Rebecca Sealy, South entrance On weekends, floor mats are being rolled up and used to block open the doors.  These mats are not always placed back in their proper place after the weekend 11/29/04   In progress
6th street in front of the Medical Fraternity House Oleanders are crowding the sidewalk, making a narrowed walking area 11/29/04 12/1/04 This is not UTMB property but they have been notified through Student Life of the concern and advised to trim the oleanders.
1902 Harborside Building (Lipton Tea) Carpet in hallway on 1st floor in creating an uneven walking surface 11/29/04 12/22/04 FOAM has tried many times to solve the problem and is still actively looking for a solution.
Texas Ave and 6th Street Stop sign removed from the intersection 11/30/04 11/30/04 Parking was unaware the sign was missing and will take care of the issue today.
Hallway in front of main Cafeteria Floor is extremely slick 11/30/04 12/1/04 As per Jason Botkin:

The hallway entrances that lead into the cafeteria have mats placed by the exit door, and a mat placed by the dietary kitchen entrance. This floor is policed by EVS employees through out the day shift (approximately 4-5 times) and it is deep cleaned with machines on 2nd shift (approximately 9pm or 10pm). The deep cleaning of this floor includes a scrub of the floor and a buffing of the floor. The floor itself and the wax we use are slip resistant products.

I do understand that things might be spilled or rain might be brought in from the outside that also could make the floor slippery. We would hope that anyone who sees this or discovers it would call the "CLEAN" line (x 25326) to have it cleaned up. We try and respond to spills and slippery spots within 15 minutes. There are also floor signs at the front information desk and by Starbucks that can be placed in a slippery spot once reported.

MRB Construction barricades are being blown across the street and almost hitting pedestrians 12/1/04 12/1/04 As per FOAM:

The barricades belong to the contractor who is working for the city. Work is finally progressing on the trolley so hopefully the barriers will not be up for long. Since the contractor is on site now, the contractor will be more aware of any barricades causing a hazard.

Patient Care Areas Staples in patient charts are poking/scratching employees 12/1/04 12/2/04 EHS has talked with Healthcare Epidemiology and they feel there is no valid concern with this issue. 

More details about specific locations and incidents would be necessary to determine any site-specific risks associated with this concern.

Hospital areas Halls are being cleaned at shift changes 12/1/04 12/1/04 As per Jason Botkin

"We have already addressed this issue and moved the floor cleaning back a few hours.  We are also working on having the majority of our floor care done on 3rd shift. "

Between Administration building and John Sealy Hospital The pebbled surface sidewalk  is slippery during wet weather or heavy dews 12/1/04   FOAM is aware of the problems with this surface and has stopped installing it on new projects.  Existing surfaces are being left as is.
Old Shrine Burns Only elevator was out service, denying ADA access for upper floors 12/3/04 12/15/04 FOAM is working on a procedure to require signage directing persons on alternate ADA accessible access.
Old Shrine Burns Elevator stopped not level with the floor 12/8/04 12/15/04 Those responsible for elevator repairs were contacted about the concern
Construction near Physical Plant Building (#21) Metal plates on ground covering holes in ground  create possible trip hazard 12/16/04 1/18/04 Calvin Sims, the Construction Inspector for OFPC has discussed the situation with the contractors on site who are responsible for the area.
M lot and Harborside Dr. Security arm hit an employee 12/30/04 1/10/05 Security arms are located in areas for vehicular traffic.  Pedestrian walkways are provided for employee use.
