Central Stairs in Children's Hospital	Construction in stairwell was not blocked off, warning signs missing, and a concrete drill was left unattended during lunch on a landing.	2/14/06	2/15/06		Contractor has disciplined persons responsible for abandoned concrete drill. Warning signs have been posted on stair doors on all floors.
S2 stairwell in McCullough, first floor	Hand rail is loose. Trash is present in stairwell	2/14/06	2/15/06		Handrail was repaired by FOAM Maintenance. Trash was removed from stairwell.
School of Nursing 1st floor, all	Floor is very slick and has resulted in several student and employee injuries.	2/14/06	2/15/06		Floor concerns are to be addressed in the current 1st floor renovation project.
Healthcare Areas	Infection control measures not fully being complied with	2/14/06	2/14/06		Concerns were forwarded to Pam Falk (Healthcare Epidemiology), Carla Kunz (Risk Management Specialist), and Bud Cherry (Director for Perioperative Nursing).
Rebecca Sealy, West end staircase	Stair door has a knob instead of a staircase	2/14/06	2/21/06		Area was examined. Existing hardware is within code, but will be upgraded during the renovation of the West building.
Grass in front of Parking Lot D	Grass area becomes wet, has rocks and holes, and is a possible trip hazard	2/14/06	2/16/06		The existing grass area serves as a shortcut to buildings West of the parking lot. Area has existing appropriate and available pedestrian walk ways to serve the area.
2nd floor Basic Sciences stairwell exit into MRB walkway	The metal plate serving as a junction between the two buildings is loose.	2/14/06	2/22/06		Repaired by FOAM Maintenance
Front entry of John Sealy Hospital (The Towers)	Floor is slick at the main entrance to the building	2/21/06	IP		Environmental services has been contacted and we are waiting on a reply.
Poison Control Center 3.112 Trauma Building	Existing leak of over 1 year has not been fixed with the possibility of mold	2/21/06	2/22/06		The office manager for the area has no knowledge of such a problem. FOAM Maintenance has found no evidence of mold. FOAM has contacted a waterproofing company to water test the area.
1902 Harborside (Lipton Tea) 1st floor door leading to old loading dock area.	Existing brace on door fell on an employee's head.	2/21/06	3/16/06		FOAM Maintenance has resolved the concern
Access to Rebecca Sealy Hospital	Rebecca Sealy Hospital is not easily accessed by persons in wheelchairs crossing from John Sealy Hospital. There are dips in the surface and ramps/sidewalks are not smooth.	2/14/06	IP		Person handling ADA concerns has been contacted.
Clinical Sciences Bldg	3rd floor Women's restroom has a door swing which creates a hazard of being hit when exiting the restroom.	2/14/06	3/1/06		FOAM has reported that reversing the door is not a viable option due to cost.
Rebecca Sealy Hospital	Report of brown water, most common on Mondays.	2/16/06	2/23/06		FOAM Maintenance has to shut water down due to construction. Due to this, the water will be brown until the sediment has been flushed from the pipes. Running the faucets until the water is clear is the recommended fix.
Rear ramp of McCullough	The lip on the plywood ramp is causing a trip hazard to employees and carts	2/22/06	8/1/06		An offset threshold has been installed
UTMB Vehicles	Employees traveling in UTMB vehicles around campus not using seat belts	2/23/06	5/2/06		The Safety Management Committee is working on a solution/training for the concern.
UHC entrance from parking garage	Doors open out and are hitting persons not aware of the doors' function	3/8/06	3/9/06		EHS has placed a warning label on the doors. National Fire Code requires these doors open outward for proper egress from the building.
Parking Garage #2	Loose wires on the floor coming from a hole	3/23/06	3/24/06		EHS has rolled the wires and placed them inside the 3" pipe. This wire as abandoned and not charged.
Market and 6th St/8th St	Motorists are threatening the bicyclists who ride on campus	7/12/06	7/12/06	H	Report was forwarded to UTMB Campus Police
Clinical Science elevators	Middle elevator in main bank of elevators dropped about one foot while in operation.	7/18/06	7/31/06	H	Elevator was removed from service 7/20/06 until repaired on 7/31/06
AMPCO parking lot intersection	Stop sign has been moved back from the intersection due to recent road re-routing	7/18/06	7/31/06	L	This intersection is not UTMB property. UTMB will contact the city and let them know of the problem.
11th and Mechanic Intersection	Cars are running intersection and almost hitting pedestrians. Stop signs are hidden by parked cars and there is no crosswalk painted.	7/25/06	7/26/06	M	A crosswalk at this location will be included in the planned ADA project for this area. Currently crosswalk signs are being installed at the location.
Harborside Crosswalk near Lot C	Crosswalk signals are not functioning properly	7/27/06	8/8/06	M	Mike Dricks with the City of Galveston reports the problem has been resolved.
Children's Hospital	Trip hazard from bubble type door stops located in the nurses station	8/2/06	8/3/06	M	Door stops were removed
New driveway poured between Brackenridge and Clay Halls	Exposed pipes (duct bank) which had original concrete removed with the new concrete not covering all the existing pipes.	8/2/06	8/3/06	M	The exposed pipes will be covered and the area brought back to grade when work on the telecom wires located in the duct bank is complete.
Rebecca Sealy Hospital	Report of accidental activation of fire alarm system	8/6/06	8/18/06	L	Area employees have been counseled in the appropriate use of the Near Miss Hazard Reporting form and the Fire Alarm Response Report form.
Surgical Research Building	Piping extending from the wall by the dog room is very sharp.	11/14/06	11/15/06	M	The pipe ends were covered with red tape to add visibility. Maintenance was notified of the problem.
Moody Library Steps	A long section of bricks was reported to be loose on the main steps leading up to the Library plaza.	12/7/06	12/7/06	M	Maintenance was notified and repairs should be in progress.
