Bethel Hall South west entrance	Wheelchair ramp wood is loose, rotting and becomes slick when wet.	10/26/2007	11/20/2007		East maintenance will rebuild the ramp.
ARC	Women's shower area is cluttered and has loose/missing tiles which can cut feet when stepped upon and are a trip hazard.	10/1/2007
Elevators on Campus	Elevators under maintenance or repair have no clear markings to stop people from entering.	9/28/2007	10/5/2007		Contractors have been told of our concerns and are now using barricades for these elevators.
Clinical Science building entrance	New landscaping of area seems to be poorly lit at night	9/21/2007	9/24/2007		The area was inspected when dark by Tim Schilling with West Maintenance. The area is currently sufficiently lit, but additional lights are scheduled to be installed during October.
Nurses stations/triage rooms at the USP	Narrow three legged desks that are unstable when leaned upon.	9/17/2007	9/20/2007		This a Texas Department of Criminal Justice (TDCJ) outlying prison and not under the jurisdiction of UTMB. The person was referred to the Authority Having Jurisdiction for TDCJ.
2nd floor Trauma Center	Dirty or broken beds/gurneys are located at the end of the corridor	9/12/2007	10/1/2007	L	The ER policy is that equipment is not to be left in the corridor. Occasionally ambulance workers will leave the back boards there to be picked up on the next run. ER staff is working with the ambulance workers to correct this issue.
Sidewalk in front of Old Red	Area floods after new landscaping	9/11/2007	9/12/2007		Area drain needs lowered and will be repaired once the Project Punch List for the area is complete
Old Red exterior steps	Remaining steps need the non skid strips placed at the top of the stairs	9/7/2007	12/18/2007		Area maintenance has confirmed plans to re-apply the non-slip coating to the stairs as the weather allows.
John Sealy Hospital (Towers)	UTMB rugs at entrance pose tripping hazard. New rugs are not flat when they are installed	9/6/2007, 10/30/07	9/7/2007		Jason Botkin has been notified. The vender has been instructed to ensure the rugs are flattened down when they are replaced. Housekeeping will monitor the area as well.
Market street from 12th to Old Red	Bird droppings on the sidewalk	9/6/2007	12/17/2007		Per West Area maintenance: We have been cleaning the area out in front and back of Old Red, and the Student Center. The birds being relocated from the front of the hospital are moving to new locations. We just added to our existing contract to have the birds chased from Old Red , Graves, UHC parking Garage, North side of MRB and MMN. As areas come up, we then add them on.
North Side Building 17 (Research Expansion Building)	The street has standing water and slippery mud. Possible trip/fall hazard	9/6/2007	10/15/2007		Area has been dug up and the cause of the standing water repaired.

Texas Ave. and 6th Street	Cars are turning left (against traffic flow direction) to short cut into the parking garage instead of going around the circle.	7/31/2007	8/31/2007		Per UTMB Police Captain Turner, a UTMB Police Officer has been posted in the area and tickets issued to violators.
Pebble Sidewalk on Market St. between 8th and 9th streets	Pothole/uneven sidewalk are a possible trip hazard	6/5/2007	6/15/2007	L	The area has been reported to the Maintenance department of Shriner's Hospital

Parking Plaza	Handrail on stairs is rusting and rubbing off onto people's hands	3/21/2007	5/7/2007	L	The handrail is made of stainless steel and is very rust resistant. The rust stains are a result of the finish and suppose to be an architectural feature of the structure.
Sidewalks around John Sealy Hospital	Reports of slippery and smelly conditions due to the bird poop in the area and the water used to clean it every morning.	3/14/2007	4/15/2007	L	FOAM has returned to methods to discourage the birds from roosting in these entrance areas in the evening. This appears to be have been successful in reducing the numbers of birds in these high traffic areas. Slip hazard signs are placed wherever and whenever sidewalks are being cleaned.
UHC entrance from parking garage	Report of the automatic single door opening and striking a person on the head	1/23/2007	1/30/2007	L	Item has already been addressed. Existing yellow sticker warns that it is an automatic door. Door also has large red lettering advising persons of the door swing direction.
The Residence	Report of possible trip hazards in the parking lot and at the ramp	1/23/2007	1/30/2007	L	The area was inspected and no areas of concern were noted. Surface was level with a slightly rough texture.
Ampco Parking	New landscaping blocks pedestrian exit from garage	1/18/07; 3/12/07	3/12/2007	M	Per Don Sexton, a purchase order was issued to install an appropriate sidewalk through the shrubs.
