To obtain a replacement diploma when the original has been lost or destroyed, students must provide a signed, written request, along with the following information: date of graduation, degree awarded, name at time of graduation, Social Security number, a check or money order in the amount of $30.00 payable to UTMB, and the address where you want the diploma mailed. The replacement diploma is issued using the current diploma design, wording, and signatures. The replacement diploma will be sent certified mail and will need to be signed for. The processing time is 3-4 weeks.
