About the Process

Since you benefited from federal student loans while attending UTMB, you are required to complete an Exit Interview. A hold may be placed on your academic records here at UTMB which could affect your graduation until you properly complete the Exit Interview process.

The Exit Interview process is as follows:

• View your repayment information for your Direct Loan at https://www.dl.ed.gov/borrower/BorrowerWelcomePage.jsp and view repayment information for your Federal Direct Consolidation Loan (if applicable) at www.loanconsolidation.ed.gov. You may also access applicable forms related to your DL by clicking on the “FORMS” link. Print your loan information. (1-888-877-7658)

• View your federal student loan history at National Student Loan Data System (NSLDS) www.nslds.ed.gov/nslds_SA/. Print your loan information. (1-800-730-8913)

• If you received Federal Institutional loans (Primary Care, Federal Perkins, Nursing Student Loan, Nursing Faculty Loan and Loans for Disadvantage Students) while attending UTMB, you must return a signed copy of the disclosure statement/agreement to our office. Logon to Affiliated Computer Services (ACS) https://www.acs-education.com/CS/Jsp/general/home.jsp to review/print your institutional loan history.

• Complete the Exit Interview form. Information will be required regarding your, parents, 2 personal references and your expected employment. If your parents are deceased, provide a next of kin. THIS IS NOT OPTIONAL. If you do not provide the information or if any of the requested information is missing, your Exit Interview will not be considered complete and you will be notified by email to provide the information that is required.

Once we have received your completed Exit Interview form and/or disclosure statement/agreement (if applicable), your HOLD will be removed and the Exit Interview Process will be complete. NOTE: These forms may be faxed, scanned/email or sent via postal mail
