Membership Application
For your convenience and security, the membership application for the UTMB Faculty Lounge is offered as a downloadable file in Adobe Acrobat (.pdf) format. (Get the free Acrobat Reader.) Forms are also available in the Faculty Lounge and in the Department of Catering and Special Events.
  • Please download and fill out the required fields in the form. A credit card number is required and will be kept on file by UTMB Department of Catering and Special Events.
  • Remember to sign and date it.
  • Completed forms may be delivered to the Faculty Lounge (G.216 Old Red) or to the UTMB Department of Catering and Special Events (1.702 Rebecca Sealy)
  • Completed applications may also be mailed to Campus Route 0184
