Entry Form for Annual Drawing

Once a person has accumulated three cards, an entry into UTMB's annual drawing may be submitted on his or her behalf. This form must be completed by one of the following:

• Employees – Submitted by immediate supervisor
• Students – Submitted by Office of Campus Life
• Volunteers – Submitted by Volunteer Services

A confirmation email will be sent to the honoree and the supervisor or appropriate person upon submission.
