Once a person has accumulated three cards, an entry into the drawing may be submitted on their behalf. This form must be submitted by one of the following:
Employees - submitted by Immediate Supervisor
Students - submitted by Office of Campus Life
Volunteers - submitted by Volunteer Services
Upon submitting this entry a confirmation email will be sent to the
honoree and the supervisor or appropriate person.
