Changes of Local Street Address

It is critical that your correct local street address is on file with UTMB and the Offices of Enrollment Services or Diversity and International Affairs at all times. Your year end tax reporting document (Form W-2) will be mailed to this address if it cannot be delivered to you personally by your employing department. Also, important communications relating to your employment with UTMB is sent to this address. In addition, there are special requirements for international students and scholars to report changes in local street address to the Department of Homeland Security. Failure to do this in a timely manner is considered a violation of the terms of your United States visa status. If you plan to change your local address, or have already moved, please read and follow the instructions below carefully to ensure that you have complied with the law and UTMB requirements.

All UTMB Employees – All UTMB employees must change their own address information in the People Soft Human Capital Management (HCM) system. To do this:

  1. Go to the UTMB “intranet” home page at http://www.utmb.edu
  2. Click on the “HR/Fin Systems” link on the left side of the page
  3. Under “Human Capital Management” click on the “HPRD-Login” link
  4. Log into People Soft HCM using your network user name and password
  5. Click on the “Self Service” link
  6. Under “Personal Information” click on “Home and Mailing Address”
  7. Your current home address should appear
  8. Click on the yellow “Edit” button to the right of your address
  9. Type in the necessary changes and click “Save”
  10. Once this has been done, your local street address will be available to all departments on campus that are authorized to access this type of information.

For UTMB purposes, you may use a Post Office box address. However, you must still share your physical street address as indicated below in accordance with Department of Homeland Security requirements.

UTMB F-1 Students - You must notify UTMB Enrollment Services, Ashbel Smith Hall (Old Red) 1.206, (409) 772-9823 within ten days of any change in your local street address to ensure proper reporting in SEVIS, and change your address on line at: https://sisweb.utmb.edu/sp/utmb/student. If you are a UTMB employee, you must also change your address in People Soft HCM as indicated above.

UTMB J-1 Students and Scholars - In addition to changing your home address in People Soft HCM (see above), complete our J-1SEVIS Compliance Questionnaire and provide it to: Office of Diversity and International Affairs, The University of Texas Medical Branch, Rebecca Sealy Hospital, 1.120, Route 0106.

J-1 Medical Residents and Fellows Sponsored by ECFMG - In addition to changing your home address in People Soft HCM, ECFMG sponsored Exchange Visitors must provide changes in contact information to ECFMG by accessing OASIS (On-line Applicant Status and Information System).

F-1 and J-1 Individuals Sponsored Under the Visa Program of Another College or University - If you are completing "F-1 Optional Practical Training" , "J-1 Academic Training", or performing activities at UTMB while holding F-1 or J-1 visa status under the visa program of another college, university, or facility you must: 1) change your address in People Soft HCM (see above), and 2) send your change of address to the person who signed your visa document at the address indicated your I-20 or DS-2019 form.

Other Temporary Visa Holders- The Department of Homeland Security requires that you complete an "AR-11" form within ten days of changing your local street address. This form is available on the USCIS website. Be sure to also change your address in People Soft HCM (see above.)

Other Temporary Visa Holders Required to Undergo "Special Registration"- If you have been registered with the Department of Homeland Security under the "Special Registration" or "NSEERS" programs, you must complete an "AR-11SR" form within ten days of changing your local street address. This form is available on the USCIS website. Be sure to also change your address in People Soft HCM (see above.)
