UTMB J-1 and J-2 visa holders must complete SEVIS Questionnaire

 

Effective August 1, 2003 this office must report certain information electronically to the federally administered Exchange Visitor Information System (SEVIS) about J-1 visa holders and their J-2 dependents sponsored under the UTMB J-1 visa program. Incomplete or inaccurate information reported to SEVIS could affect your legal immigration status in the United States. If you are a J-1 visa holder sponsored under the UTMB J-1 visa program, you will be asked to complete our "J-1 SEVIS Questionnaire" to report changes to your address, employment, and citizenship information as well as to request a Form DS-2019 for yourself or a dependent.

The form is now available for completion on line.  Please fill in the requested information, print the completed form, sign and date it at the bottom of page (1), attach your current proof of health insurance (if other than UTMB employee benefits and/or Trawick International Medical Evacuation & Repatriation insurance), and return the completed form to the Office of International Affairs, Administration Building 2.202, Route 0140. Information about J-2 spouses and children must be included on the same questionnaire with that of the principle J-1 visa holder, even if the J-2 dependent is also a UTMB employee.

NOTE: You are sponsored by UTMB if the J-1 Program Number in section (2) of your DS-2019 is P-1-05972. (If you hold a Form IAP-66 the number is P-1-5972.) Do not complete this questionnaire if your J-1 visa status is sponsored through another university or agency. Instead contact your J-1 program sponsor to ensure proper reporting to SEVIS occurs.
