Pre-Arrival Data Sheet - Must be completed by all foreign nationals
seeking visas status for UTMB and provided to prospective UTMB supervisor who will forward it to the Office of International Affairs
with the visa request.
Reimbursement of "Expenses Only" for International Person
Determining Appropriate Visa Status To Receive Payment for Services:
Several factors must be considered in determining the appropriate visa classification as well as the correct
method of payment for an international visitor. Follow the steps outlined below to determine whether a prospective
visitor should be treated as an employee or independent contractor and whether a B-1 "Visitor for Business" visa
or another visa classification will be required.
- Complete the Employee/Independent Contractor Verification Form
- If person is Independent Contractor visiting UTMB for nine days or less:
- If person is Independent Contractor visiting UTMB for longer than nine days, a J-1 visa is required.

Have visitor complete Office of International Affairs Pre-Arrival Data Sheet and return it to sponsoring department with copies
of visa documents reflecting previous visits to the U.S.
- Prepare J-1 Sponsor Letter and, if the visitor holds the equivalent of an MD degree, No Patient Contact Letter.

- Forward completed documents referenced in 3a and 3b above to your Office of International Affairs representative.
- Click here to complete additional forms required to arrange for Independent Contractor payments.
- If person is an Employee, a J-1 or other employment-related visa classification is required and he/she must be appointed
in the payroll system.

Have visitor complete International Affairs Office Pre-Arrival Data Sheet and return it to you with copies of visa documents
reflecting previous visits to the U.S.
- Upon receipt of completed Pre-Arrival Data Sheet and attachments, contact your
International Affairs Office representative for assistance.
