Clinical Staff Invited to Participate in Patient Safety Survey

Please take a few moments to participate in the AHRQ Culture of Safety Survey.

Your thoughts and opinions are needed to find out what is going well and what can be improved upon. The survey includes items that measure areas or composites of patient safety culture, including: communication openness, feedback and communication about error, frequency of events reported, handoffs and transitions, management support for patient safety, nonpunitive response to error, organizational learning-continuous improvement, overall perceptions of patient safety, staffing, supervisor/manager expectations and actions promoting safety, teamwork across units and teamwork within units.

The survey will be emailed to you - look for an email from the Office of the President; click on the appropriate link for your area; answer the questions, click save and submit.

If you did not receive a survey but think you should have, please contact Stephanie Ramos at pssurvey@utmb.edu.

Thank you for devoting 10 minutes of your valuable time to complete this very important and anonymous survey.
