• Where to take the samples for processing.
• Turnaround time expected from time the lab receives the specimen.
• Special Instructions
• The specimen required
• The volume desired and the minimum acceptable volume
• The container used to transport the specimen to the laboratory.
• Reference ranges
• The last time the information was reviewed and who did the review.
