Autopsy/post-mortem examination: (000-0000)

Clinical Indication:

Determination of cause of death, assessment of response to treatment and co-existing disease states.  Answer family or clinician's questions, and research.

Methodology:

 Post-mortem Examination, Necropsy

Performed:

Monday-Friday from 8:00 AM to 5:00 PM (autopsies will not start after 2:00 pm)
Autopsy Suite 518 Clinical Science Building
 

Turnaround Time:

Provisional Autopsy Diagnosis within 24 hrs
Final Autopsy Diagnosis within 30 working days for routine cases and 60 working days for complicated cases.
 

Specimen Requirements: Refer to Decedent Affairs Website

Request Form: Form 5012 (rev 7/08) Authorization for Post-Mortem Procedures;
Authorization for Private Autopsy 5012-PR (rev 11/07).

Causes for Rejection: Lack of authorization by legal next-of-kin, telephone permit without fax confirmation, Medical Examiner jurisdiction.

Clinical Information:

If applicable, state clinical information that is required to be provided with specimen.

Reference Range:

By report (reports may vary based on instrumentation, patient age and sex)

Note:

Physician requests permission from the legal next-of-kin and completes the Form 5012, Section 4. For private autopsies and consults, call the Autopsy Service at (409) 772-2810, go to Autopsy Service website.

Autopsy with gross and microscopic examination and laboratory tests as indicated. Full or limited examination, with or without CNS (brain/spinal cord).

When ordering tests for which Medicare or Medicaid reimbursement will be sought, physicians should only order tests that are medically necessary for the diagnosis or treatment of the patient.  Components of the organ or disease panels may be ordered individually.  The diagnostic information must substantiate all tests ordered and must be in the form of an ICD-9 code or its verbal equivalent.
