Request Form: Designated Donation Request signed by consulting M.D., recipient, and donor

Patient Preparation: Informed consent must be signed by patient

Collection: Drawn by Blood Bank Staff.

Storage Instructions: Unit will be held for 7 days after anticipated use.

Causes for Rejection: Donor must meet allogeneic (volunteer) donor criteria

Availability: Donors are accepted 8:30 a.m. – 5:00 p.m., Monday-Friday; Blood Bank John Sealy Annex, Room 1.210

Turnaround Time: 3 working days

Special Instructions: Donation must be scheduled with Blood Bank Donor Room Staff.

Volume: 450 mL

Minimum Volume: 300 mL

Container: Blood Bag (CPDA-1)
