PLATELETS (007-0099)  "PLATELETS"

Synonyms: Platelet Packs, Platelet Transfusion, Pooled Platelets

CPT 4 Code: P9019

Order Mnemonic: BILL PLT

Order Includes: Platelet concentrate consists of the platelets separated from the whole blood collected from a random donor suspended in 50 mL of plasma and contains at least 5.5 x 1010 platelets.

Request Form: Blood Bank Division Primary Request Form

Patient Preparation: Patient must have a wristband for checking against component container label prior to administration.  A recent (i.e., within the same 24-hour period) platelet count should be completed before platelets are ordered.

Collection: Specimens must be labeled with the patient's name and UH#.

Storage Instructions: Storage is at room temperature with agitation for up to 5 days.  Pooled platelets must be transfused within 4 hours of pooling.

Causes for Rejection: Requisition will be questioned if patient has a platelet count within the hemostatic range. 

Availability: Samples are accepted 24 hours per day; Blood Bank, Room 1.180, JSA

Turnaround Time: 30 minutes

Special Instructions: Type and Screen required during current hospital visit.  Patient must meet acceptable transfusion criteria.  One hour post transfusion platelet count to be drawn on patient.  All requests must include requesting physician’s name and ID number, patient's name, UH #, patient location, clinical information/diagnosis.  Request form must be filled out completely and all information must be legible.

Volume: 7 mL Lavender-topped tube for initial ABO and Rh type and antibody screen

Minimum Volume: 3 mL blood

Container: Lavender-topped tube (EDTA)
