Test Includes: current Type & Screen required for each hospital admission

Lab: Blood Bank

Request Form: Blood Bank Division Primary Request Form

Storage Instructions: products will be dispensed with a Blood Bank cooler if more than 2 units are requested at a time

Causes for Rejection: Acceptable transfusion criteria not met. 

Availability: Samples are accepted 24 hours per day at the Blood Bank lab, McCullough 5.102

Turnaround Time: 1 hour

Special Instructions: Type and screen required during current hospital visit.  All requests must include requesting physician’s name and ID number, patient's name, UH #, patient location, clinical information/diagnosis.  Request form must be filled out completely and all information must be legible.  Plasma must be transfused within 24 hours after being prepared for patient.
