PACKED RED CELLS, transfusion (0007-0131) "PACKED RED CELLS, transfusion " Synonyms: Packed Cells; Red Blood Cells, Human; Red Cell Concentrate; Red Cells, Packed; Transfusion, Type and Crossmatch Order Mnemonic: BILL RBC Test Includes: ABO and Rh type, antibody identification when screen is positive, antibody screen, crossmatch Applies To: Donor Blood Lab: Blood Bank Request Form: Blood Bank Blood Component Request Patient Preparation: The patient must have an identification wristband Storage Instructions: Stored in Blood Bank refrigerator only until issue. When unable to infuse after issue, blood must be returned to Blood Bank within 30 minutes Causes for Rejection: Gross hemolysis, drawn in SST/serum separator tube, requisition not signed by phlebotomist. Availability: Samples are accepted 24 hours per day; Blood Bank, Room 1.180, JSA Turnaround Time: 2 hours Special Instructions: All requests must include requesting physician’s name and ID number, patient's name, UH #, patient location, clinical information/diagnosis, identity of phlebotomist and date and time collected. Request form must be filled out completely and all information must be legible. Crossmatched blood held for 3 days only.
