TYPE AND COMPATIBILITY TEST (007-0049, 007-0096, 007-0047, 007-0159)  "TYPE AND COMPATIBILITY TEST"

Synonyms: Compatibility Testing, Crossmatch, Incompatibility Testing, Type and Hold for # Units of Blood, Type and Screen for # Units of Blood.

Test Order Mnemonic: TX

Test Includes: ABO and Rh type, antibody screening, crossmatch

Applies To: Donor Units; Major Screen; Packed Red Cells; Transfusion

Request Form: Blood Bank Division Primary Request Form

Patient Preparation: Patient must have a wristband identification.

Collection: At the patient's bedside, label the specimen with the patient's ful name, UH #, date, and initials of collector on sample.  Information should be obtained from wristband.

Causes for Rejection: Gross hemolysis, drawn in serum separator tube, requisition not signed by phlebotomist, label not legible, or specimen not properly labeled.

Availability: Samples are accepted 24 hours per day in the Blood Bank lab, McCullough 5.102

Turnaround Time: STAT: 1 hour; Routine: 2 hours

Special Instructions: All requests must include requesting physician’s name and ID number, patient's name, UH #, patient location, clinical information/diagnosis, identity of phlebotomist and date and time collected.  Specimen must be labeled with patient full name, UH # and Date of Collection. Specimen must be transported to the laboratory in a biohazard specimen bag with request form in the pocket of the bag.  Request form must be filled out completely and all information must be legible.  Blood is held for 3 days only.
