Request Form: Write in on Routine Chemistry Form 1- Body Fluid

Storage Instructions: 2-8°C (refrigerated) if not received in the lab within 4 hours after collection

Causes for Rejection: Specimen is too viscous for sampling

Availability: Samples are received 24 hours per day at Sample Management, 5.136 McCullough.

Turnaround Time: Routine: 24 hours; STAT: 1 hour

Special Instructions: List source of fluid on request form.
