Request Form: Routine Chemistry Form 1

Collection: Routine venipuncture; AM collection preferred

Storage Instructions:  Red-topped Tube: Centrifuge, remove serum from cells, refrigerate until delivered to lab; if delivery is delayed by more than 24 hours, freeze @ -20°C.

Causes for Rejection:  gross hemolysis or lipemia and blood drawn in any anticoagulated tube.

Availability: Samples are received 24 hours per day at Sample Management, Room 5.136 McCullough.

Turnaround Time: 24 hours. Test is performed Monday-Friday.

Special Instructions: The test can be ordered only once daily. Additional requests require the approval of the laboratory director.

Specimen: Blood

Volume: 7 mL blood

Minimum Volume: 0.5 mL serum

Container: SST (gel barrier), 7 or 10 mL red-topped tube (see storage instructions)

Reference Ranges: Euthyroid: 0.76-1.23, Hypothyroid: <0.76, Hyperthyroid: >1.23; values on pediatric patients not established
