HEMOGLOBIN ELECTROPHORESIS (089-3106)

Synonyms: Hgb-Elect, Hb-Elect

CPT 4 Code: 83020

Test Order Mnemonic: HGBELECT

Test Includes: Sickling test, hemoglobin identifications, hemoglobin quantitations, hemoglobin F%, hemoglobin A2%, interpretation of hemoglobinopathy by Pathologist.

Applies To: Aids in diagnosis of patients presenting with certain hematological abnormalities (i.e., thalassemia, anemias, abnormal RBC morphology)

Request Form: Hematology B

Patient Preparation: If patient is to receive RBC transfusion, collect before transfusion.

Collection: Routine venipuncture

Storage Instructions: Specimens not delivered to the laboratory within 2 hours should be refrigerated (stored at 2 - 8°C).

Cause For Rejection: Collected in wrong tube, clotted, identification error

Availability: Samples are accepted 24 hours per day Sample Management, 5.136 McCullough.

Turnaround Time: Routine testing is batched on Tuesday/Thursday.  Maximum 7 day turnaround time.  STAT testing is available ONLY if needed prior to RBC transfusion.  STAT turnaround time is 4 hours, Monday-Friday if received BEFORE 2 pm.  STAT testing is not available on weekends or holidays.

Specimen: Blood

Volume: Pediatric: 1-3 ml; Adults: 5-10 ml

Container: preferred EDTA (Lavender-top tube) ; accepted Heparin (Green-top tube)

Reference Ranges: Hemoglobin A2 = 1.3-3.3%; Hemoglobin F% £ 1.5

Test Methods: Test Battery Includes tube solubility (Sickle Screen) and High Performance Liquid Chromatography (HPLC); with Isoelectric Focusing (IEF): Microchromatography; Alkali Denaturation and Citrate or Cellulose Electrophoresis when indicated. 

Interfering Substances: Transfusions
