GRAM STAIN: (088-0012)

Clinical Indication:

 Stain for observation of cellular elements and microorganisms from patient samples.

Test Mnemonic:

 C GS (The gram stain is included in many bacterial culture requests without separate order)

Methodology:

Microscopic examination of Gram stained slides

Performed:

 Clinical Microbiology

Turnaround Time:

 Stat - 1 hour, Routine 8-12 hours

Specimen Requirements: Patient Preparation: Cleanse infected site

Collect: Collect the sample - aspiration, biopsy, fluid, swab using aseptic technique

Specimen Preparation: Sterile container

Storage/Transport: Room temperature, refrigerate if transport is delayed

Stability: 48 hours

Causes for Rejection: Improper collection, swabs received more than 72 hours after collection

Note:

A direct gram stain is routinely performed on specimens from most body sites if two swabs are submitted or if fluid, aspirate or tissue is submitted.

Order a gram stain separately when it is needed STAT or from a source that does not routinely include a gram stain.

Order C WBC for fecal leukocyte exam is requested.

When ordering tests for which Medicare or Medicaid reimbursement will be sought, physicians should only order tests that are medically necessary for the diagnosis or treatment of the patient.  Components of the organ or disease panels may be ordered individually.  The diagnostic information must substantiate all tests ordered and must be in the form of an ICD-9 code or its verbal equivalent.
