Test Order Mnemonic: Syphilis screen: RPR; Tertiary Syphilis Testing: SYPG

Test Includes: All samples are screened with the RPR test. If the screening test is positive, RPR titer and confirmation tests are performed. A screen and titer are performed on previously positive patient samples- confirmation is not required.

Lab: Serology

Request Form: Microbiology/Serology

Collection: Routine venipuncture, SST (serum separator tube)

Storage Instructions: Centrifuge and store at 2-8°C.

Causes for Rejection: Hemolysis, submission of tubes other than SST

Availability: Samples are accepted 24 hours per day at Sample Management, 5.136 McCullough.

Turnaround Time: Test performed daily Monday - Friday.   Confirmation tests are performed two times per week.

Special Instructions: Guidelines for testing patient samples: If a patient has previously been positive for RPR (without confirmation), it is recommended to wait two months before testing again. If the RPR is negative, it is recommended to wait one month before testing again. For patients diagnosed with syphilis (i.e., positive RPR and confirmatory test), the CDC recommends that response to therapy be assessed by testing the RPR (looking for a 4-fold decline in titer) after 6 months. Tests on samples collected less than the above time frames will be cancelled unless approved by a Microbiology Director or designee. Exception: Specimens from Labor and Delivery are tested if seven days have elapsed since the last test. Tests are performed on all obstetrics patients regardless of previous results.  If there is no previous patient data, the requested tests are performed.
