SYPHILIS IgG SCREEN (088-0236)

Synonyms: , Syphilis Test, Treponema pallidium IgG  Replaces: Rapid Plasma Reagin, RPR

CPT 4 Code: 86780, RPR titer 86593 (088-0046), Medicare NCD  

Test Order Mnemonic:  Syphilis Testing: SYPG

Test Includes: All samples are screened with the Syphilis IgG  test. If the screening test is positive or indeterminate, RPR titer and confirmation test (TP-PA) are performed. RPR titer is performed on previously positive positive patients samples, but confirmation is not required.

Lab: Serology

Request Form: Microbiology/Serology

Collection: Routine venipuncture, SST (serum separator tube)

Storage Instructions: Centrifuge and store at 2-8°C.

Causes for Rejection: Hemolysis, submission of tubes other than SST

Availability: Samples are accepted 24 hours per day at Sample Management, 5.136 McCullough.

Turnaround Time: Test performed daily Monday - Friday.   Confirmation tests are performed two times per week.

Special Instructions: Guidelines for testing patient samples:  For patients diagnosed with syphilis (i.e., positive IgG and confirmatory test), the CDC recommends that response to therapy be assessed by testing the RPR (looking for a 4-fold decline in titer) after 6 months. Exception: Specimens from Labor and Delivery are tested if seven days have elapsed since the last test. Tests are performed on all obstetrics patients regardless of previous results.  If there is no previous patient data, the requested tests are performed.

Volume: 2ml serum

Container: SST (gel separator); red top - remove serum from clot before submission unless immediate transport is possible

Reference Ranges: Non-reactive

Test Method: IgG - Chemiluminescent immunoassay (BioPlex 2200); RPR - (latex agglutination)
