Lab: Surgical Pathology

Request Form: Surgical Pathology Laboratory Tissue Consultation (Requisition Form 86)

Collection: Surgical excision, punch biopsy, curettage

Storage Instructions: Submit biopsy in 10% neutral buffered formalin in a container labeled with the patient's first and last names, unit history number, and specimen identification.

Causes For Rejection: Insufficient identification, incomplete request form, contaminated container.

Availability: Deliver to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex, Monday-Friday, 8:00 AM-5:00 PM. After hours, specimens may be left in the refrigerator adjacent to the Surgical Pathology Laboratory that opens to the corridor. 

Turnaround Time: Routine: 24 hours; STAT: Must be received in Surgical Pathology Laboratory by 12:00 noon for same day turnaround.

Special Instructions: Request form must include ICD-9 code, physician's name, ID code, signature, clinical history and diagnosis.

Container: Pre-filled containers with 10% neutral buffered formalin are provided by Surgical Pathology Laboratory, Room 2.186, John Sealy Annex.

How To Order: Submit biopsy specimen with proper form to the Surgical Pathology Laboratory, Room 2.186, John Sealy Annex.
