BIOPSY TISSUE EXAMINATION (except gastrointestinal, kidney, liver, lymph node) (98305)

Test Includes: Gross and microscopic examination with diagnosis.

Applies To: Tissue removed from patients at surgery or biopsy.

Lab: Surgical Pathology

Request Form: Surgical Pathology Laboratory Tissue Consultation (Requisition Form 86)

Collection: Surgical excision, punch biopsy, curettage

Storage Instructions: Submit biopsy in 10% neutral buffered formalin in a container labeled with the patient's first and last names, unit history number, and specimen identification.

Causes For Rejection: Insufficient identification, incomplete request form, contaminated container.

Availability: Deliver to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex, Monday-Friday, 7:30 AM-4:00 PM. After hours, specimens may be left in the refrigerator adjacent to the Surgical Pathology Laboratory that opens to the corridor. Surgical Pathology personnel will pick up specimens twice per day in outpatient clinics Monday-Friday.

Turnaround Time: Routine: 24 hours; STAT: Must be received in Surgical Pathology Laboratory by 12:00 noon for same day turnaround.

Special Instructions: Request form must include ICD-9 code, physician's name, ID code, signature, clinical history and diagnosis.

Specimen: Tissue

Container: Pre-filled containers with 10% neutral buffered formalin are provided by Surgical Pathology Laboratory, Room 2.186, John Sealy Annex.

How To Order: Submit biopsy specimen with proper form to the Surgical Pathology Laboratory, Room 2.186, John Sealy Annex.
