Request Form: Request for Intra-Operative Consultation Form and patient's chart are required. A completed Surgical Pathology Laboratory Tissue Consultation Requestion form 86.

Availability: Contact the Surgical Pathology staff at X22855, Monday-Friday, 7:30AM-4:00 PM; also available after hours, on weekends and holidays by on-call staff.
