Availability: Deliver to Surgical Pathology, Room 2.180, John Sealy Annex, 8:00 AM-5:00 PM.

Turnaround Time: 2 days

Special Instructions: Report from outside hospital or lab must accompany slides. Request form must include ICD-9 code, physician's name, ID code, signature, clinical history and diagnosis.

How to Order: Submit outside material with proper form to Surgical Pathology, Room 2.180, John Sealy Annex.
