Lab: Surgical Pathology

Request Form: Surgical Pathology Laboratory Tissue Consultation Requisition Form 86.

Collection: Needle biopsy, fine needle aspiration, surgical excision.

Storage Instructions: Tissue is placed in tissue transport medium composed of Culture Medium 199 and fetal calf serum which is available in the Surgical Pathology Gross Room, extension 22855. Specimen should be labeled with patient’s name, unit history number and specimen identification and be delivered immediately to the Surgical Pathology Laboratory, John Sealy Annex, Room 2.186. Short term storage 4°C if necessary. DO NOT FREEZE.

Causes for Rejection: Improper fixation, sample not noted appropriately, improper labeling, incomplete form, contaminated container.

Availability: Contact the Renal Pathologist on-call. Deliver to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex, Monday-Friday, 8:00 AM-5:00 PM. Also available after hours, on weekends and holidays by renal pathologist on call.

Turnaround Time: 3 working days

Special Instructions:

  • Obtain transport medium from Surgical Pathology Gross Room, ext. 22855
  • Contact Renal Pathologist on call
  • Patient information form must be completed
  • Request form must include ICD-9 code, physician's name, ID number, signature, clinical history and diagnosis

Specimen: Tissue

Container: Vial with tissue transport medium composed of Culture Medium 199 and fetal calf serum obtained from the Surgical Pathology Gross Room, ext. 22855, Room 2.186.

How To Order: Submit specimen with proper forms immediately to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex.
