IMMUNOFLUORESCENCE – biopsy (98346)

Request Form: Surgical Pathology Laboratory Tissue Consultation (Requisition Form 86)

Collection: Tissue biopsy

Storage Instructions: Tissue biopsy is placed in tissue transport medium composed of Culture Medium 199 and fetal calf serum which is available in the Immunohistochemistry Laboratory extension 28051. Specimen should be labeled with patient’s name, unit history number and specimen source to be delivered immediately to the Surgical Pathology Laboratory. Short term storage at 4°C, if necessary. DO NOT FREEZE. Do NOT place in formalin.

Causes for Rejection: Specimen placed in formalin or other fixative, sample not stored appropriately, insufficient identification, incomplete request form (pink sheet).

Availability: Deliver to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex, Monday-Friday, 7:30 AM-4:00 PM.

Turnaround Time: One working day.

Special Instructions:

  • Obtain transport medium from laboratory prior to biopsy.
  • Contact Surgical Pathology, X22855
  • Request form must include ICD-9 code, physician’s name, ID code, signature, clinical history and diagnosis.

Specimen: Tissue biopsy

Container: Vial with tissue transport medium composed of Culture Medium 199 and fetal calf serum obtained from Immunohistochemistry Laboratory, extenstion 28051.

How to Order: Submit specimen with proper form immediately to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex. Contact Surgical Pathology, X22855 (409-772-2855).
