Request Form: Surgical Pathology Laboratory Tissue Consultation (Requisition form 86)

Collection: Needle biopsy of kidney

Storage Instructions: Needle biopsy of kidney is placed in tissue transport medium composed of Culture Medium 199 and fetal calf serum which is available in the Immunohistochemistry Laboratory, extension 28051.  Specimen should be labeled with patient’s first and last names and unit history number and be delivered immediately to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex.Short term storage at 4C, if necessary. DO NOT FREEZE. Do NOT place in formalin.

Causes for Rejection: Specimen placed in formalin or other fixative, sample not stored appropriately, insufficient identification, incomplete request form

Availability: The Renal Pathologist on call MUST be contacted when kidney biopsy is pending. Deliver to the Surgical Pathology Laboratory, Room 2.186, John Sealy Annex, Monday-Friday, 7:30 AM-4:00 PM. Also available after hours, on weekends and holidays by renal pathologist on call.

Turnaround Time: Transplant: one day; Routine: 3 days

Special Instructions:

  • Obtain transport medium from laboratory prior to biopsy.
  • Contact Renal Pathologist on call.
  • Patient Information Form must be completed.
  • Request form must include ICD-9 code, physician’s name, ID code, signature, clinical history and diagnosis.

Specimen: Needle biopsy of kidney

Container: Vial with tissue transport medium composed of Culture Medium 199 and fetal calf serum obtained from Immunohistochemistry Laboratory, extension 28051.

How to Order: Submit specimen with patient's chart and proper forms immediately to Surgical Pathology Laboratory, Room 2.186, John Sealy Annex. Contact Renal Pathologist on-call.
