Stethoscope If you are a medical provider or representative, you are responsible for reporting notifiable diseases to the UTMB Montgomery County Health Department.

You can now complete and submit influenza and influenza-like-illness reports online! Simply log on to www.montgomerycountyhealth.net and register as a provider. You may also submit flu reports using the Flu Reporting Form.

To report a case of varicella (chickenpox), please use the Varicella Reporting Form.

To report any other infectious disease, please print the Infectious Disease Report.

Once completed, disease reporting forms may be faxed to 936.539.9272 or emailed to siibrahi@utmb.edu.
