- Download Project registration form: for first time users, this form allows us to gather information about your laboratory and your plans and ideas, it will help us prepare to help you. Must be signed by user and PI (if other than user)
- Download User Contact Information: must be completed by each user . Needs to be updated if user moves to another lab.
- Download OMC policies and fees: must be reviewed and signed by user and PI. This is to make sure the user and the PI are familiar with the OMC rules and operational fees. An updated form will be sent to all users and PIs each time a major change to the rules or fees occurs
- Download FRS authorization form: must be completed by PI and should list the account information to be used for billing purposes and should list all users authorized to use this account. A new form must be completed before an account expires. Also a new form must be completed to add new users or to add a new account for the lab.
