New Users
In order to use the services or resources of the OMC, all new users need to complete the following registration forms, we also recommend to request a meeting with either Dr Paulucci or Dr Vergara for a preliminary discussion about your plans and needs.
- Download Project registration form: for first time users, this form allows us to gather information about your laboratory and your plans and ideas, it will help us prepare to help you. Must be signed by user and PI (if other than user)
- Download User Contact Information: must be completed by each user
- Download OMC policies and fees: must be reviewed and signed by user and PI. This is to make sure the user and the PI are familiar with the OMC rules and operational fees.
- Download FRS authorization form: must be completed by PI and should list the account information to be used for billing purposes and should list all users authorized to use this account.
