Walls of Honor Registration Form

Please provide a completed form and picture allowing your service honoree to have their picture and service information displayed on both the UTMB online web site and on the bulletin board located in John Sealy Hospital's "white hallway" near the staff elevators.

INSTRUCTIONS: Complete the form below and then email the picture to Pastoral Care. Other options are to send the picture by Campus Mail to route #0201, or to bring the picture for scanning to the Pastoral Care office located at 1.220 John Sealy Hospital located by the Gift Shop.
