Pathogenesis of Diabetic Glomerulopathy
Diabetic glomerulopathy is the leading cause of renal insufficiency in the United States. Our work focuses on the pathogenesis of lesions that are characteristic of the advanced stages of diabetic glomerulopathy. An understanding of how these lesions develop is a prerequisite to their prevention and treatment.
Our initial studies suggest that focal mesangiolyses (FM) are the precursors of Kimmelstiel-Wilson (KW) nodules. KW nodules are large lesions capable of impairing glomerular function. FM's appear to result from hemodynamic disturbances and to develop into KW nodules within a relatively short period of time, possibly a few months. This rapid evolution of structural lesions is contrary to prevailing opinion, but does fit with recent observations that diabetic nephropathy can be relatively rapidly improved by various antihypertensive regimens. Our current studies suggest that insudative lesions (deposits of plasma lipids and proteins in glomerular arteriolar and capillary walls) can impair glomerular function and also appear to develop rapidly. - Selected Publications
