The Diagnosis of Blood Disorders

As a hematopathologist, I am involved in the diagnosis of blood diseases including leukemias, lymphomas and bleeding disorders.

In collaboration with the cancer and leukemia group B (CALGB), I have reviewed and verified the diagnosis of hundreds of cases with acute leukemia and myelodysplastic syndromes (MDS).

A major area of my interest is the MDS. This group of diseases is characterized by abnormal production and maturation of blood cells. MDS tend to have a characteristic morphology associated with abnormal function of individual cells. The myelodysplastic syndromes are associated with bone marrow failure in addition to increased risk for the development of acute leukemia. MDS in children are even more challenging. The French American British (FAB) and the World Health Organization (WHO) classifications of MDS do not apply very well to the pediatric group. Inherited bone marrow failure syndromes may evolve into MDS and this could pose a diagnostic problem.

In collaboration with the Shwachman Diamond Syndrome International Foundation and with the National Institute of Health, we are studying patients with this rare syndrome and other bone marrow failure syndromes to assess the use of various makers to predict and correlate with the development of MDS. The outcome of MDS development in these syndromes is not clear. We are applying morphologic, cytochemical, immunohistochemical and flow cytometric methodologies in this study. This study is being expanded to acquired cytopenias, including acquired aplastic anemia.
