		At the end of the rotation, submit your portfolio.   Your performance on this rotation will be evaluated through your written and verbal answers to the questions and on your reflection of the experiences.  You will be assessed on the six  ACGME competencies, specific to this rotation. You are responsible for completing the following:  □   complete the Curriculum Review Confirmation in New Innovations □ e-mail 14 modules (emailed to Niebuhr and other faculty):      AGE MODULES   TOPIC MODULES   Prenatal, 2-wk 2-mo   Developmental Screening   4,6,9,12 month   Health Promotion/Developmental Promotion   15,18,24--month   Breastfeeding   Preschoolers   Childcare   Middle childhood   Dental health       Immunizations       System-based Practice       Medical Screening       Nutrition   □ Developmental screenings □  Developmental Screening Reflections  (submit by email) □    Developmental screening log (turn in portfolio at end of rotation) □  Denver II scoresheet  (documentation of negative findings should have been made in medical record) □  collection of Draw-a-Person's for patients over 3 □ New Innovations Procedure Log:  Documentation of developmental screenings is required in the procedure log for your residency program.  Take time during this rotation to enter these developmental screenings into your procedure log. □ Pairing with a Pediatrician Reflections (submit by email) □ ECI LAUNCH Reflections (submit by email) □ Rotation Evaluation  (submit by email) □  evaluation of faculty through New Innovations   last edited 9-17-2012
