  1. to demonstrate your skills at writing patient records.
  2. to reflect on the impact of patients and patient care on yourself

Write about one patient encounter from this rotation which significantly impacted your learning experience.

1.  We suggest you answer prepare your response in Word , then cut and paste into an e-mail. 
2.  Submit answers by e-mail to Course Directors  (Gayani Silva, MD, gsilva@utmb.edu and Virginia Niebuhr, PhD, vniebuhr@utmb.edu )  

Due by Friday of Week 2.  

  Provide the following

  1. a complete clinic note using whatever format you are most comfortable with

  2. a reflection about this patient/family:   what touched you about this case? what did you learn about children, pediatrics, or yourself from this case?
