        -Staff must print the score page (last screen) for documentation.
        -Passing score = 100%
  2.  Validation Tools- These are the check-off guidelines for each bedisde or point of care test, 3 forms are
        available depending on the POC test :-)
      -Kits Val Tool  (pregnancy, UA Dip, guaiac, Strep)
      -POC Monitors Val Tools (glucose, HemoCue, Hematocrit)
      -Low Volume Tests Val Tool (pH-Nitrazine)
 3.  Competency (CHECK-OFF) forms- worksheets for documenting blind sample results)
      -UA Dip
      -Miscellaneous (Pregnancy, pH-Nitrazine, hemoglobin, hematocrit)

THE COMPETENCY TEST SCORE PAGE, VALIDATION TOOL, AND WORKSHEETS MUST ALL BE STAPLED AND PLACED IN EMPLOYEE FILE.
