4. I am going to read a name and address to you. When I finish I would like you to repeat the name and address back to me.  This is not a person that you know.  This is a check on your memory.

       Score3 – 25-35 secs; 2 – 20-24 or 36-40 secs; 1 –15-19 or 41-45 secs; 0 – < 15 seconds or > 45 secs

8.         Please watch what I do with my hand.  Notice the sequence of movements, beginning with this one (demonstrate fist, edge, palm for 2 seconds each).  I’m going to do that again, so watch carefully (demonstrate a second time).  Now you try that.

 9.   When I say the word “red” I want you to raise your finger like this (demonstrate).  Then you can put it down (demonstrate).  When I say “green” don’t do anything (demonstrate holding fist in a closed position).
