• We are located on the first floor of the McCullough Building in room 1.228. Some people may know it as the old Emergency Room. Our phones numbers are (409) 772-2531 and (409) 772-2533. You can also fax consults to (409) 747-0025

• If you are unable to keep your appointment please call (409) 772-2531 during regular business hours to re-schedule.
   

• Our hours of operation are Monday through Friday from 8AM to 5PM
   

• When the patient comes for a clinic visit please bring all medications that the patient is taking.
   

• Please bring your insurance card.
   

• For pain medication that one of the radiation oncologist has prescribed you please bring the prescription bottle.
   

  • If you are calling for refills related to your oncological treatment please:
     

    • Allow 1 business day for refills
       

    • If calling for a phone refill please have the pharmacy name and phone number with area code, the patient's name and phone number and also all medication information
       

• For regular medications please see your primary care physician (IE: Blood pressure medications, diabetes medications, etc and obtain refills from them
