Project 3 - Community Health Improvement Project
Methodology
Follow a process of identifying community stakeholders and other constituencies that will bring focus to the cancer health disparities within specific areas of each county:
- Determine the most valuable starting point to address cancer, with the ultimate goal of creating a comprehensive program to meet all aspects of community need
- Use Community Cancer Report Cards, recently completed ACS community profiles, and other resources to assess the cancer burden within each county for efficient planning, perhaps to the neighborhood level
- A Framework for Community-Based Cancer Prevention & Control (from the Georgia Department of Health Resources) will be devised by the community stakeholders to identify priority targets to reduce or eliminate cancer health disparities
Planning process will:
- Map community assets and resources by assessing existing programs and resources that could be extended to adopt a cancer focus
- Determine areas where efficiencies and economies of scale might improve programs and services to reduce or eliminate cancer health disparities
- Enable stakeholders to assess gaps in service, explore best practices, and implement programs that will that will match local assets with local needs for improved cancer prevention and control services
